• Perform comprehensive patient assessments, including vital signs monitoring, blood glucose checks, and diabetes-related evaluations. Assist in diabetes management, including insulin administration, medication management, and patient education. Support physicians and specialists during consultations, medical procedures, and treatments. Ensure accurate documentation of patient records, treatment plans, and follow-ups in the electronic medical record (EMR) system. Complete and send visit summaries to patients in a timely manner, ensuring clear communication of treatment plans, medication changes, and follow-up instructions. Questionnaires Collection and completion Data Collection & Research Collect and record patient health data, including blood glucose levels, weight, blood pressure, and other relevant health metrics. Analyze patient data trends to assist in early identification of complications and treatment adjustments. Utilize wearable health devices and digital monitoring tools to track patient progress and enhance personalized care. Support data collection for clinical research studies, ensuring accurate recording and reporting of relevant health information.
